MEDICARE - CHRONIC CONDITION MANAGEMENT PLAN
Who is eligible for a Chronic Disease Management Plan?
If you have a chronic medical condition, your General Practitioner (GP) may create a Chronic Condition Management Plan. These plans were formally called ‘Enhanced Primary Care’ plans (EPCs) and ‘Chronic Disease Management Plans’.
These management plans are designed to help more effectively coordinate the care you need from your GP and other health care providers. There is no list of eligible conditions, your GP will determine whether a plan is appropriate for you.
Once a plan is in place, it will be regularly reviewed by your GP. This is an important part of the planning cycle, where you and your GP check that your goals are being met and agree on any changes that might be needed.
