Who is eligible for a Chronic Disease Management Plan?
If you have a chronic medical condition, your General Practitioner (GP) may suggest a GP Management Plan (GPMP). If you also have complex care needs and require treatment from two or more other health care providers, your GP may suggest Team Care Arrangements (TCAs) as well. These plans were formally called ‘Enhanced Primary Care’ plans (EPCs).
These plans will help more effectively coordinate the care you need from your GP and other health care providers.
There is no list of eligible conditions, your GP will determine whether a plan is appropriate for you.
Once a plan is in place, it will be regularly reviewed by your GP. This is an important part of the planning cycle, where you and your GP check that your goals are being met and agree on any changes that might be needed.
Can I use my Chronic Disease Management Plan on Physio?
TCAs that include Physiotherapy can be used at Hartwell Physiotherapy. Your physio will provide your GP with information regarding your condition, management and progress.
Will there be an out-of-pocket cost?
Yes, the Medicare rebate only covers part of your Physio fee. For all Physio appointments, Pensioners and Health Care Card holders receive a 10% discount.
We process the full fee on the day of your appointment and accept the Medicare rebate on the spot.
